New! Submit changes to the claims coordinator designation online through SORM's RMIS secure website



Required:
When a change is made in the designation of the person acting as the claims coordinator for a particular agency, department, office, board, commission, etc., that change should be communicated to SORM as soon as possible.

Filing Deadline:
The form must be received by SORM no later than the 10th calendar day after the change.

Completed by:
Either the new or old claims coordinator, or other designated agency personnel.
