SORM is planning a phased implementation that will begin in September and be completed by the end of 2011.
SORM is making every effort to make this change as seamless as possible. To that end, each agency must designate a Health Care Network (HCN) contact and enter that person's information into SORM's online Risk Management Information System (RMIS). Your designated HCN contact will receive periodic updates from SORM, as well as e-mails and videos that can be viewed by all staff for additional information about the network and their responsibilities.
The five basic responsibilities of employing agencies are:
Distribute the Employee Network Notification Packet provided under the CompKey+ HCN Information Packets table shown above. This can be done either electronically or in paper format. The packet must be distributed to all current employees. The packet will be available in both English and Spanish. All newly hired employees will need to be provided with the Employee Network Notification Packet and acknowledgment form within 3 days of their date of hire.
Receive back from the employee the acknowledgment form. The acknowledgment form must be:
Signed by the employee;
Returned to the employer and;
Maintained by the employer.
The acknowledgment form does not need to be sent to SORM but may be requested by SORM, if there is a dispute as to when, or if, an employee was provided the Employee Network Notification Packet.
Post a copy of the Employee Network Notification Packet at each place of employment.
Remind employees who suffer an injury or illness at work by providing them with an additional copy of the Employee Notification Packet.
Track the date and method of delivery of the Employee Network Notification Packet, including to whom it was delivered and the location of the delivery.
