SORM is requesting customer service feedback to measure customer satisfaction with our service. The survey results will help SORM measure the quality of its support, identify ways in which SORM can improve the customer experience, and ensure we more effectively meet customer needs.

If you need to report an incident of workers’ compensation fraud, please contact our Special Investigations Department at (877) 445-0006. Non-workers’ compensation fraud, waste, or abuse should be directed to the State Auditor’s Office Fraud Reporting form, or calling their Fraud Hotline at 1-800-TX-AUDIT (1-800-892-8348).
